Certain life events can affect your SPARC benefits. See what actions to take and what to keep in mind as you navigate life’s changes. If you have a qualifying life event, benefit changes must be made on SmartBen (Forever 21 employees) or Dayforce (all other SPARC companies) within 30 days. You may also be able to make changes to your benefit elections if SPARC makes a substantive plan change to a program during the year. QLE Documentation

You must process the qualified life events (QLE) and provide proper documentation within 30 days of the QLE in order to obtain coverage.

Forever 21 Employees

  1. Log into your enrollment system, click the "Begin Enrollment" icon on the homepage, and select the appropriate next step option from New Hire, Annual Enrollment and Life Event buttons. 
  2. Click on each benefit to enroll or update your selections.
  3. Once your elections are complete, each benefit will have a green light next to it. Click "Elect & Continue" to proceed.
  4. Review your selections for accuracy, click “Continue” when prompted, and enter your initials to agree to your enrollment selections.

All Other SPARC Employees

  1. Log into your enrollment system and go to Benefits, then Forms.
  2. Fill out and submit the Life Event Declaration form and attach the proper backup documentation.
  3. Once approved by the Benefits Department, under the Benefits tab, you will see a “Pending Enrollment.” Click “Start Enrollment.”

The effective date for your change will be the event date of the QLE. Remember to review and update your beneficiaries as needed when making changes to your benefits.

New child
(birth, adoption, or foster)

This is a qualified life event. When you experience a qualifying life event, such as adding a new child to your family, you have 30 days to make benefits changes related to your event. Visit SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) to adjust your benefits coverage.

  • Review SPARC’s leave policies such as Maternity Leave and Paid Parental Leave.
  • Notify your manager about the timing of your leave.
  • Call Voya at 888-973-3652 to begin processing your leave.
  • Get a Social Security number for your new child, if an application has not yet been submitted.
  • Add your new child onto benefits coverage through the life event process in Dayforce or SmartBen etc.
  • Consider updating your life insurance beneficiary information on Voya.
  • Consider updating your retirement plan beneficiary information.
  • Update your will. Get assistance from ARAG if you’re enrolled in the Legal Services plan.
  • Notify your manager of your return to work.
  • Access free, confidential counseling through the Employee Assistance Program (EAP).
  • If you receive a court order requiring you to add coverage for a dependent child, make sure to add them to your medical plan.
Getting Married
(or new domestic partnership)

This is a qualified life event. When you experience a qualifying life event, such as getting married, you have 30 days to make benefits changes related to your event. Visit SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) to adjust your benefits coverage.

  • Update your personal information, if necessary, in SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands).
  • Complete the Domestic Partner Affadavit and supporting documentation, if applicable.
  • Change or update your benefits coverage on SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) within 30 days.
  • Update your life insurance beneficiary information on Voya.
  • Update your retirement plan beneficiary information.
  • Update your will. Get assistance from ARAG if you’re enrolled in the Legal Services plan.
Getting divorced
(or legally separated/ending a partnership)

This is a qualified life event. When you experience a qualifying life event, such as getting divorced, you have 30 days to make benefits changes related to your event. Visit SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) to adjust your benefits coverage.

Death in the family

This is a qualified life event. When you experience a qualifying life event, such as a death in your family, you have 30 days to make benefits changes related to your event. Visit SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) to adjust your benefits coverage.

Loss or gain of benefit coverage
(you or a dependent)

This is a qualified life event. When you experience a qualifying life event, such as gaining or losing other benefits coverage, you have 30 days to make benefits changes related to your event. Visit SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) to adjust your benefits coverage.

  • Update your benefits coverage on SmartBen (Forever 21 employees) or Dayforce (all other SPARC brands) within 30 days.
Leaving the company
  • Review the Separation Guide (Storeline) or Separation Guide (Corporate) to understand how this change impacts your benefits and compensation.
  • Notify your manager.
  • Return all company property.
  • Make decisions about your ongoing benefits coverage. You can expect to receive COBRA information mailed to you at home after your benefits coverage ends.
  • Convert supplemental life insurance to an individual policy, if desired.
  • Access free, confidential counseling through the Employee Assistance Program (EAP).
  • Determine eligibility for unused vacation pay.
  • Contact your retirement plan administrator Voya to learn about your options.

This is a qualified life event. After you’ve left the company and are no longer eligible for SPARC benefits, you are eligible to enroll in other coverage, such as COBRA coverage or your spouse’s benefits. Ask your spouse’s employer for their enrollment process and deadline.
